When to use this form

Use this form when you need official school records for a student, such as immunizations, report cards, attendance, enrollment verification, IEPs, or discipline notes. It is helpful for transfers between districts, scholarship and sports eligibility checks, or employment and background screening. If you only need an academic transcript, send a separate request using the Transcript request form. Families moving schools can pair this with a Transfer request form so we can send records to the new campus without delay. If a third party (like a college, attorney, or employer) should receive the file, include signed authorization from the student or guardian. For items outside the cumulative file, such as letters of enrollment or custom documents, submit a Document request form.

  1. Student's full legal name and date of birth?

    These identifiers uniquely match the student across systems and past schools. Accurate details reduce search errors and prevent sending records for the wrong person.

  2. Current school, grade, and last school attended (include student ID if known)?

    This helps us locate the correct file quickly, especially for transfers. Including a student ID speeds us through duplicate names and archive pulls.

  3. Which records do you need and for what time period?

    Being specific (for example: immunizations, attendance reports, IEP, disciplinary log, or enrollment verification from 2021-2024) keeps the scope focused and turnaround fast. If you need a letter or non-record item, use the Document request form.

  4. Where should we send the records, and who is the authorized recipient?

    Providing the recipient's name, organization, email or address prevents failed deliveries. If the recipient is not the parent/guardian or the student, attach a FERPA Waiver form so we can release records.
